Samuel has personally attested to its World Series game use by inscribing the garment "Juan Samuel/#16/My 83 World Series Shirt" in black marker. The white pinstripe zipper-front jersey features the team's "P" logo on the left breast and the number 16 on the right breast and reverse. "Samuel" is lettered upon a white nameplate on the reverse, directly above the number. 1983 Phillies 100th Anniversary patch appears on the left sleeve. The name "Samuel" is chain-stitched in black upon a white strip tag in the collar. Samuel appeared in three World Series games, but his only two home appearances came in Games 4 and 5.
