Left shoulder has an Avalanche patch, and the right shoulder an NHL All-Star Minnesota 2004 patch. On the front is an NHL patch and a CCM patch is found on the verso below the collar. There is an NHL MeiGray Group label reading "NHL game Worn Jersey 2003-2004 Season" on the inside and the stamped number "B11769". It is signed on the verso #19 in bold black sharpie by Sakic. There is also an embroidered NHL and NHLPA logo on the bottom back side. Light use. NHLPA LOA.
