This amazing piece of Frank Sinatra history is a questionnaire Ol' Blue Eyes fillied out in 1943 for Columbia Records four days before he signed with the label. It is dated May 28, 1943; Sinatra signed with Columbia on June 1, 1943. Sinatra was already a burgeoning star at the time this was filled out. He was an idol to the "bobby soxers," as teenage girls of the time were called, and his partnership with Columbia propelled his career to new heights. Sinatra hand wrote/signed his full name "Frank Sinatra" twice at the top of the form, listing it as both his professional and legal name(s), and also signed on the bottom. Other handwritten responses include information such as his address, date of birth, marital status and more. Sinatra would go on to become one of the most legendary performers in American history, with an iconic voice that enraptured listeners and acting chops to rival his singing prowess. Of Sinatra's 56 Top 20 albums, 42 reached the Top Ten and six made the #1 position. The New Jersey native also won an Academy Award for Best Supporting Actor in 1954. The questionnaire measures 8.5x13" and has a horizontal crease near the bottom to make it easier for Sinatra to sign. There are a few minuscule holes at the top and two staple marks on the sides; it is otherwise in EX condition and comes with a JSA LOA.
